Health Requirements

The Massachusetts Department of Public Health and Hospitals School Health Program requires complete Health Records on every child before entering school.

THE REQUIREMENTS FOR CHILDREN ENTERING PRESCHOOL IN SEPTEMBER:

*Complete up to date immunizations:
4 doses DPT
3 doses Polio
1 doe MMR
3-4 doses HIB
3 doses Hepatitis B (for all children born on or after Jan. 1, 1992)
1 dose Varicella (or M.D. verification of disease)
*Results of Lead Screening
*Signed Physical Exam done within 12 months of starting school
*TB Risk Assessment (usually done by M.D. during physical exam)
*Health History

You should be aware that a copy of the physical exam with the above immunizations is necessary before the first day of school in September. Your child will not be allowed to start school until all medical requirements are met.
